MI Choice services are for adults 18 and older who have nursing-home level of care needs. They want to live at home or in another residential setting instead of a nursing home. The residential setting could be a home, assisted living, adult foster care or homes for the aging.
The MI Choice program offers on-going care management that is responsive to the needs of the individual. For example, when additional hours were provided so a spouse could be away for an all-day meeting. In another case, evening hours were added so a spouse could continue to attend a monthly activity he or she has participated in for several years.
Care plans are flexible to best serve the individual and their family. In the case of a woman with dementia, she found the initial care plan with different providers coming in twice a day too overwhelming. The plan was adjusted to one care provider coming in once a day for a longer period of time. This has worked very well for the client.
Individuals who meet income, asset, and level of care criteria, can receive a range of services and supports. Options might include help with bathing, dressing and transferring, or light housekeeping, laundry, home-delivered meals, specialized equipment, caregiver relief, adult day care, nursing services, non-medical transportation, a personal emergency response system, electronic medicine dispenser, or help moving from a nursing home.
